Maryland Bridge Between Two Implant Crowns

I have two dental implants and just lost the tooth that is between them. My dentist is tinkering with the idea of putting a Maryland Bridge between them. Have you had experience with this type of restoration? Does it work okay to put a Maryland Bridge between two crowns?

Maryland Bridge

I don’t think this is going to be your best viable solution for a few reasons. First, while a Maryland Bridge sounds easy to place on the surface, it is actually a bit more complicated than most dentists think. The best material to bond it to is tooth enamel. In your case, that isn’t an option. Your dentist will be trying to bond it to porcelain, which is harder.

Secondly, for a Maryland Bridge to have any security they need a groove etched into the adjacent teeth for the wings. Because you have a dental crown, there is a good chance it the groove will end up taking things to the metal base which makes it even harder to retain.

Ideally, your dentist would have anticipated that with two dental implants you might one day need a third. Then he or she could have made the implants parallel to one another and used screw-retained crowns. This way, if you lose the middle tooth, it becomes a simple matter of turning your two crowns into a dental bridge.

Seeing as that is not possible, I would first suggest seeing if you can fit a third dental implant in there. That would give you the most secure retention.

Tanda Pearl Ionic Teeth Whitening

I have professional teeth whitening trays that my dentist said to wear for an hour a day to get optimum whitening. That has been aggravating my TMJ. I recently read an ad for Tanda Pearl Ionic Teeth Whitening. This one says you only have to wear it for 5 minutes twice a day. That appeals to me and I think it would be easier on my jaw. Are you familiar with this system? Would you say it works and, equally important, is safe?

The Concept

The Tanda Pearl system works using the same process that you’d get with the teeth whitening your dentist offers, though milder. The gel goes in a tray that you wear.

The Tray

There is a big difference in the Tanda tray and the tray you get at your dentist. With a professional tray, your dentist custom fits it to your bite, with a separate tray for each arch. This should be the type of tray you have. With the Tanda system, there is only one tray that you hold in by biting down on it. You could not sleep with it and certainly wouldn’t want to be seen with it in public, unless you enjoy weird stares. Plus, holding the tray in place will aggravate your TMJ Disorder significantly more.

The Time

My guess is they only have you wear it for 5 minutes because any longer than that would be painful. There is no magic to either that amount of time or the time your dentist prescribed. The longer you wear them the more the gel has time to work. If one hour a day is too hard for your jaw, then wear the trays for less time. I’d also talk to your dentist to make certain your trays are properly fitted to your bite. It shouldn’t be uncomfortable. In fact, usually, all a patient has to worry about with extended wear is the sensitivity of their teeth.

The Safety

As to whether or not it is safe, I would say yes. I don’t see anything that would make it unsafe. It has a mild teeth whitening strength, that combined with the short amount of time you wear it, there is little chance of sensitivity.

However, I think you’d still get better results just sticking with the teeth whitening you’ve already paid for with your dentist. I would lower the amount of time you are wearing the trays to minimize the aching in your jaw. I’d also make sure your TMJ problems are being addressed and not just diagnosed.

A Pretty Smile after 23 years in Dentures

I’ve had removable dentures for 23 years. A friend of mine recently got dental implant dentures. These look so much nicer than my teeth and she can eat anything she wants. Is it too late for me to have a pretty smile? I’ve spent my lifetime embarrassed by it.

I’m sorry you have been embarrassed by your smile. The great news is it is never too late for you to get a beautiful smile. The key is in the dentist you choose. Doing a smile makeover is not taught in dental school. As a result, it is up to the dentist as to how much post-doctoral training they receive, if any, in creating beautiful smiles. Additionally, there is some natural artistry that is necessary as well, as with any art.

As to the dental implants themselves, because of the length of time you have been in dentures, there will be significant bone loss. That will have to be addressed in order for you to have a successful chance for true retention of your dental implants. The integration with the bone is imperative. The best way to address this is to have a bone grafting done. This will build back up the missing bone that you will need for osseointegration.

Once that is healed, you can have the implants placed. There will be a second period of healing necessary after the implants are placed. From there, you can have your dentures anchored to the new implants. This is known as implant overdentures, sometimes called implant-supported dentures.

Finding the Right Dentist

With the right dentist, you can have a gorgeous smile you will be proud to share with the world. However, choose carefully. The first thing you will want to do is make sure they have the training they need to place dental implants. Take a look at Dr. Cheek’s qualifications to get an idea of the type of training you want in your dentist.

Once you are assured they are qualified to place your implants, be certain to look at their smile gallery. This will give you an idea of what type of results they can provide you when it comes to creating a beautiful smile for you.
